Comprehensive On-Site Support

With around-the-clock staffing, including nursing, an intermediate-care facility is an ideal place to call home for individuals with complex medical and behavioral needs.

We believe that with the right support, there are no limits to what an individual can accomplish on their life journey.

One of the most important ways we encourage independence is through the cultivation of relationships. We work to help individuals build positive relationships with family members, friends, co-workers, and others they care about.

Over time, these relationships help ensure they have the natural support they need when transitioning to community-based living.

To help the relationships in their life thrive, we offer:

  • Get to and from work, religious observations, community events, and visits with family and friends with provided accessible transportation.

Organized Groups

  • Join a small group of peers and take part in local community events and activities.

Individual Interests

  • Pursue passions, enjoy new experiences, or become a volunteer; the opportunities are endless!

Transition Planning

While individuals can choose to live in an intermediate care facility for as long as they’d like, our goal is to help them build the skills they need to transition to community-based living.

Transition planning begins at intake, and continues throughout an individual’s stay. Typically this includes:

Setting individualized goals and supporting achievement

N

Building key independent living skills

N

Encouraging the development of natural supports

N

Identifying areas that need additional support

N

Working with the individual's team to plan for transition needs

Respite Services

At Park West, we also offer respite services so that families and caregivers can take a break and get relief whenever they need it most.

While on respite, individuals will have access to and benefit from all the comprehensive services that we offer. Our focus on continuity of care will help ensure a smooth transition to and from the individual’s primary place of residence.

Respite stays offer peace of mind:

Flexible Length of Stays

  • We offer stays ranging from 7 – 90 days.
N

Full Admission Option Following Respite Stay

  • If an individual likes Park West and finds an intermediate-care facility best meets their needs, full admission options may be available.
N

Private Pay Options

  • If an individual is not currently receiving services through their county-board, private pay options for respite are available.
