Request for Accommodation

To Whom It May Concern,

The Americans with Disabilities Act (ADA) affirms that people with disabilities have the same rights and opportunities as others without disabilities when they visit public places. This includes restaurants, movie theaters, gyms, museums, amusement parks, zoos, concert halls, grocery stores, and other places.

The individual sharing this information with you requires a Personal Care Assistant to ensure equal access to this business, organization, or activity because they have a disability.

We respectfully ask that you waive any admission fees for their Personal Care Assistant as a reasonable modification of your policies and procedures. This Personal Care Assistant is with the individual to support them in accessing this public place and will not benefit personally from this accommodation.

If you have questions about the rights of individuals with disabilities protected under the ADA, please visit Disability Rights Ohio for more information.

Thank you for your consideration,

Rebecca Sharp
