7.15.01 Park West – Whole Facility Evacuation
Purpose
To ensure that all individuals and staff will be provided with safe environment in the event of a severe weather condition or natural disaster that results in the need for the facility to totally evacuate the grounds. Everyone will be transported to our Corporate Offices located at 540 Industrial Mile Road. In the event that the local authorities want us to evacuate to a location of their choice, we will follow their instructions.

Procedure
Full facility emergency response evacuation drills will be conducted each year on each shift.
APARTMENT’S EVACUATION ROUTES:
Each building will evacuate to the front parking lot as follows:
B Building will evacuate through the front doors and proceed to the middle of the parking lot.
C Building will evacuate through the back doors and proceed to the right side of the parking lot (by C-3’s back entrance)
A Building will evacuate through the back doors and proceed to the left side of the parking lot (by A-1’s back entrance.
STAFF RESPONSIBILITIES
In-charge person: The Executive Director, if unavailable the Residential Systems Director.
Secure key to 540 Corporate Office
Secure credit card
Assist with and oversee evacuation procedure and progress
Maintenance Department, or if unavailable the receptionist will:
Notify each building that there is a whole facility practice drill or an emergency evacuation and give the following information to apartment staff.
Ensure that each individual has needed ambulatory devices such as
wheelchairs and walkers
Notify the designated outside liaison of the planned evacuation, (Open Door Executive Director)
Proceed to front parking lot and have staff sign evacuation in-service forms and to complete the whole facility checklist (see attached).
Apartment Staff, QIDPS and other available office staff:
Notify/alert the most independent individuals first, so they can begin to get themselves ready: give information in a clear, simple terms; tell them to dress appropriately for the weather, where to go or wait, and ask them to assist others, gather materials, etc.
Notify individuals who need more assistance and assist them with their dress and wheelchairs, etc.
Assemble all individuals in living room/front hall area. Take a head count/roll call to ensure that everyone is accounted for.
Make sure that everyone including staff are dressed to go outside.
Proceed to front parking lot.
Take another head count/roll call to ensure everyone is accounted for.
Nursing Staff:
Identify individuals with special evacuation needs.
Assist individuals and give staff special instructions for individuals with special needs: Tube feeding; Colostomy; serious medical conditions.
Assemble emergency medical materials: First aid/medical cart; medications etc.
Assist with helping staff if needed in dressing, mobility assistance, helping individual cope with stress and anxiety, etc.
Lead DSPs
Secure vehicle key box
Secure Mobile phones
Assist with helping staff if needed in dressing, mobility assistance, helping individual cope with stress and anxiety, etc.
Maintenance/Housekeeping Department:
Watch and time each building’s evacuation to ensure procedure is being done in an orderly manner.
After evacuation, go through each building to ensure that everyone is out, that appliances are turned off and that needed equipment has been taken with staff.
Assist with helping staff if needed in dressing, mobility assistance, helping individual cope with stress and anxiety, etc.
Note: If this is an actual emergency evacuation, Maintenance and Housekeeping will assist all staff and individuals in the designated vehicles and then again recheck all the buildings to ensure that everyone is out.
Office Support Staff
Assist with helping staff if needed in dressing, mobility assistance, helping individual cope with stress and anxiety, etc.
The following supplies will be stored at the 540 evacuation site:
The Disaster Supply kits for each apartment
PPE and incontinent supplies
The emergency food supply and adaptive eating equipment. To ensure that the food supply is not out-dated, the emergency food supply will be rotated every six months. Other meals for individuals and employees will be provided after we arrive at our evacuation designation.
The “All Clear Signal” will be given by the Finance Director/designee for individuals and staff to return to their apartments and work locations.
If the need for total evacuation arises after normal business hours, (evenings, weekends and/or holidays), the nurse on duty will contact the Lead DSP On-call. The Lead DSP will contact the Executive Director. The Maintenance Department, Office Support Staff, Off-duty nurses and QIDPS will be contacted to come on grounds to assist. If the nature of the emergency prevents off-duty staff from getting to Park West, the local Fire Department and/or Sheriff Department will dictate further instructions to the available staff on grounds.
