6.13.1 – Hepatitis B Vaccine Declaration Form

Policy

Any employee of Open Door may receive the Hepatitis Vaccine series.  This series of injections will be made available to each employee within 30 working days of employment.

Procedure

  • All employees will attend a formal orientation program where a detailed explanation of infection control policies will be provided
  • The employees will sign a Hepatitis B vaccine declaration form during this session (Medical/nursing) of their orientation
  • The employees will be made aware that they can change their decision at any time and that their initial choice is not binding
  • If an employee does not respond to or come and receive the Hepatitis B Vaccine, it will be considered that they no longer want the vaccine and will be filed in their medical file as refused.  At any time the employee can request to resume the series.
