4.30 SCL – Informed Consent

Policy

Open Door provides all individuals with an informed consent packet (via electronic systems) upon admission or initiation of any Waiver service.  This packet is reviewed with individual and guardian by the Licensed Social Worker or designee and Open Door has tablet available to complete electronically. Consent means knowing all there is to know about something-either a program, service, or activity and then making decision based on what you know.

Before any new service starts, every individual is given the opportunity to review their options and rights.  This may be through a tour, virtual method, a meeting, or in writing.  All individuals are given the right to make choices based on accurate information provided.  A copy of individual’s bill of rights are provided annually.  This is an opportunity to bring up any questions or discuss any services that the individual does not like or ones they would like to try.

Procedure

Upon selection of Open Door as provider and annually thereafter, the individual and guardian will review the Informed Consent Packet with the Social Services department. 

The consent is available via our website opendoorcolumbus.org at anytime.  The informed consent form is used for all Open Door Waiver Services departments which includes, but is not limited to, information and obtaining consent acknowledgement in the following areas: rights’ review, grievance resolution, release of information, health/HIPPA/Hospitalization, Conflicts of interest, payment, picture release, time limitations, home health, nursing, advance directives, finances, behavior support and pharmacy review, weapons consent, appliance use, etc.
