4.29 SCL – Individual Records

Open Door must maintain the highest level of confidentiality and privacy regarding individual’s records. Open Door must comply with HIPAA law regarding protection of Protected Health Information. (refer to HIPAA policy-3.00 series)  and with federal, state, and local regulations at all times. 

Individual’s and guardians may request a copy of documents through written request. 

Open Door will store individual’s records according to culling guidelines outlined in federal, state, and local laws/regulations.  Open Door will properly shred any documents containing private information.

Any breech of confidentiality, must be immediately reported to Director or Associate Director and follow-up actions implemented including proper notifications and security of information.
