4.16 ICF – Overnight Data and Sleep Documentation

Conducting Bed Checks

In order to ensure proper supervision, the following procedure must be followed by the Direct Support Professional (DSP) every evening, only when requested by a physician:

  • Each hour the DSP will enter the individuals’ room to observe if they are sleeping, awake, disturbed, or if there is an unusual incident.
  • If individuals are easily disturbed, it is recommended that the DSP enter through the bathroom area.
  • Based on the individuals’ medical or behavioral condition, bed checks may be required to be completed more frequently as outlined in the Person-Centered Plan (PCP).
