4.11 SCL – Assessments
The individual’s team completes the Annual Person Centered Plan including assessments for each individual receiving residential services in waiver settings within 30 days of their admission or services span date and at least annually thereafter.
For Waiver settings, the assessment packet is completed by the County SSA and includes:
- Individual Specific Risk Assessment: including rights, self-determination, personal development, interpersonal relationships, social inclusion, physical/emotional well-being, daily living activities, path to employment, housing, money management, safety checklist, medical, risks/supports, and supervision.
- Self-medication assessment completed by nurse.
- During Pandemic Period, DODD guidance requires risk assessment and signing of provider assurance form for day programming. The Open Door Program Team should complete the “Return to Work” form on JotForm to notify all departments. Community employment does not apply and may never be restricted.
For waiver settings, each assessment identifies areas for growth, ADL’s, programming, or informal supports which are indicated and prioritized during the person-centered plan process by the interdisciplinary team. Formal programming and supports are then created to assist individuals with the outcomes that they want or need to work towards increased independence.
